Cosmetic Dentistry: Before and After
Van - Finally Loving Her Smile After Orthodontics
Before

After
After
Situation
Van had already invested the time and effort to straighten her teeth with orthodontics. She expected to love her smile when treatment was finished, but something still wasn’t right.
The frustrating part was that she couldn’t quite explain what was bothering her. Her teeth were straight, but when she looked at her smile, it didn’t look the way she had hoped.
What Van couldn’t easily identify was the difference in the shapes and proportions of her front teeth. An existing implant crown was bulky and blocky, while the three neighboring teeth were noticeably triangular. Individually, these details might not have seemed significant, but together they created an imbalance that kept her from feeling happy with her smile.
Experience
When Van came to us, our first priority was understanding what she was seeing and what she wanted to change. She didn’t need someone to tell her that her teeth were straight. She already knew that. She wanted to understand why she still didn’t love her smile.
As we evaluated her smile together, we were able to identify the subtle differences in tooth shape and proportion that were making such a big difference in the overall appearance.
For Van, this was an important turning point. Instead of simply knowing that something looked wrong, there was now a clear explanation and a plan to address it.
We also recognized that the gum tissue around her existing implant crown needed attention. Replacing the crown and adding veneers alone wouldn’t necessarily give us the natural result we wanted. We needed to consider how the teeth and gums would look together.
Treatment Plan
We approached Van’s treatment by planning with the end in mind. Before beginning, we determined what the ideal shapes, proportions, and relationships of her four front teeth should be.
In a single visit, we went from her existing smile to designing and 3D printing custom temporary restorations that reflected our vision for the finished result. This was a technically challenging process, especially at the time her case was completed, but it gave us an opportunity to begin transforming her smile without immediately committing to the final restorations.
And we didn’t rush the next step.
The temporary restorations allowed us to evaluate the new shapes and proportions while giving the gum tissue around her implant time to reshape. Van returned for follow-up visits, and we made refinements as needed.
Rather than expecting everything to be perfect on the first attempt, we used this phase to carefully work toward the result we wanted.
Once we were satisfied with the appearance of her smile and the surrounding gum contours, we finalized the records for her permanent restorations: one new implant crown and three porcelain veneers.
Result
Van’s transformation involved just four teeth, but the difference in her smile was remarkable.
The bulky implant crown and triangular neighboring teeth were replaced with restorations that complemented one another. Her new smile had the balance and natural appearance that had been missing, even after her teeth were straightened.
What makes Van’s story meaningful isn’t simply that she received veneers and a new crown. It’s that she had already taken steps to improve her smile and was still left feeling that something wasn’t right.
By taking the time to understand what was bothering her, designing the result before treatment, and refining the details before finalizing her restorations, we were able to address the concerns that had brought her to us in the first place.
Sometimes a smile makeover isn’t about changing everything. It’s about finally identifying the few things that need to change—and taking the time to get them right.
Tammy - A Smile Worth Waiting For
Before

Situation
Tammy had spent years feeling unhappy with her smile. As a former dental assistant, she understood dentistry better than most patients, but she had continued putting off the extensive treatment she knew she needed.
With an important birthday and a trip to Portugal approaching, she finally decided it was time to do something for herself.
Her dental concerns went well beyond appearance. One of her front teeth had fractured, resulting in infection, gum recession, and bone loss that was more extensive than what someone could appreciate simply by looking at her smile. She also had multiple teeth that required significant restorative treatment.
Tammy wanted a beautiful, natural smile, but achieving that result would require addressing both the visible problems and the underlying dental challenges.
Experience
From the beginning, Tammy was actively involved in deciding how she wanted to approach her treatment.
One of the biggest decisions involved replacing her failing front tooth. As a former dental assistant, she understood the advantages and disadvantages of both dental implants and bridges. After discussing her options, Tammy preferred a bridge.
She was comfortable with the additional attention to home care that a bridge would require, particularly because it was replacing a front tooth. She also appreciated that this approach could allow us to achieve the result she wanted in a shorter timeline.
Our job was to respect her preferences while developing a plan that would deliver a natural appearance and restore function.
Although Tammy wasn’t particularly anxious about dentistry, we incorporated IV sedation to make her extensive treatment appointments more comfortable.
Her upcoming trip to Portugal became another meaningful part of the experience. She traveled wearing her temporary restorations, already enjoying the appearance of her developing smile while we continued working toward the final result.
Treatment Plan
Tammy’s transformation involved approximately 20 teeth, making this a comprehensive restorative and cosmetic case rather than a simple smile makeover.
We planned the treatment with the final result in mind, coordinating the appearance of her teeth, her bite, the health of the surrounding tissues, and the sequence of procedures needed to achieve a predictable outcome.
The failing front tooth was one of the greatest challenges. Because the fracture had caused significant gum recession and bone loss, simply removing the tooth and placing a bridge would not have created the natural appearance we wanted.
We performed bone and soft tissue grafting to rebuild and improve the foundation for the replacement tooth. This was especially important because the area was highly visible whenever Tammy smiled.
We also designed and 3D printed custom temporary restorations. These provided restorations during healing while allowing us to prototype the final smile before committing to permanent restorations.
As her tissues healed, we evaluated the design, made refinements, and used what we learned to guide the final restorative work.
Rather than trying to complete every possible treatment at once, we staged the dentistry around Tammy’s priorities. Approximately 20 teeth were addressed during this transformation, while a few posterior teeth were intentionally reserved for treatment in a future phase.
Result
In approximately two months, Tammy went from a smile she had been unhappy with for years to a comprehensive transformation that left her ecstatic with the result.
The outcome was particularly rewarding because of the challenges involving her fractured front tooth. Despite the significant recession and bone loss present at the beginning, careful tissue management and restorative planning allowed us to achieve an exceptional esthetic result.
Her transformation involved extensive dentistry, but the finished smile looks natural and balanced. The goal was never simply to make her teeth whiter or replace what was damaged. It was to create a smile that complemented her face and gave her the confidence she had wanted for years.
Just as importantly, Tammy played an active role in the decisions that shaped her treatment. Her preference for a bridge, her comfort with the home care involved, and her personal timeline were all considered in developing the plan.
Tammy’s story illustrates that comprehensive smile makeovers don’t have to follow a standard formula. Through careful planning, digital technology, and thoughtful sequencing, we were able to complete a significant transformation in just a couple of months while leaving room for future treatment when appropriate.

Jerry - Finding a Better Way Forward
Before
After
Situation
Jerry had been a patient of ours for many years, although his demanding work schedule and frequent travel meant his dental visits were often few and far between.
Over time, Jerry became increasingly unhappy with his smile. Seeing himself on video calls made him more aware of the appearance of his teeth, and years of wear and existing dental work had taken their toll.
One particular concern was an implant that had been placed in the early 2000s. The implant and its restoration had never given Jerry the appearance he wanted, but he had been told that the result was as good as it could be.
That experience stayed with him. Jerry wanted to improve his smile, but he was understandably hesitant about pursuing more extensive treatment, especially if it meant additional implant surgery.
Eventually, he decided he was ready to make a change. He just needed a plan he felt comfortable moving forward with.
Experience
Because we’d known Jerry for years, we understood his dental history and the challenges that had made him reluctant to pursue treatment.
Rather than simply focusing on replacing individual restorations, we talked about what he wanted his smile to look like and what had been holding him back.
Jerry was particularly interested in avoiding another implant procedure. We discussed a different approach using bridgework, allowing us to address the appearance of his smile without repeating a treatment experience he wasn’t eager to undergo again.
His travel schedule was another important consideration. We wanted to develop a coordinated treatment plan that could address his concerns efficiently while minimizing unnecessary appointments.
For Jerry, moving forward wasn’t just about improving his teeth. It was about finding an approach that made sense for him after years of believing his options were limited.
Treatment Plan
Jerry’s treatment involved porcelain veneers and bridgework to improve the appearance, proportions, and overall balance of his smile.
The existing implant presented one of the greatest challenges.
It had been placed in the early 2000s, when implant planning and restorative techniques were different from what we use today. Its position made achieving a natural appearance difficult, and removing the implant fixture would have risked damaging the surrounding bone.
Rather than subjecting Jerry to a potentially damaging removal procedure, we took a more conservative approach. We removed the existing implant abutment and crown, left the implant fixture buried, and performed bone grafting to help restore the area for the planned bridge.
This allowed us to move away from the previous implant restoration and create a different solution without unnecessary surgical trauma.
We also had to account for years of wear, Jerry’s history of clenching, and the existing restorative work throughout his mouth.
By planning his veneers and bridgework together, we could improve the shapes and proportions of his teeth while creating a more coordinated, natural appearance.
The goal was not simply to replace what Jerry disliked. It was to design a smile that worked with his existing dental conditions and offered a predictable path forward.
Result
Jerry achieved a more balanced, natural looking smile that addressed the concerns he’d been living with for years.
The combination of veneers and bridgework allowed us to improve his smile while managing the challenges created by his older implant restoration, without requiring removal of the buried implant fixture.
What makes Jerry’s transformation particularly meaningful is that he had spent years believing there wasn’t much more that could be done. His previous experience had made him hesitant, and his busy schedule had made it easy to keep putting treatment off.
By taking the time to understand his concerns and approaching his dental problems differently, we were able to develop a plan that respected his preferences and gave him another path toward the smile he wanted.
Jerry’s story is a reminder that just because you’ve been told a result is the best that can be achieved doesn’t necessarily mean there are no other options.
Sometimes a fresh perspective, thoughtful planning, and a different restorative approach can make all the difference.
